Colorado Legal Services (CLS) developed these evaluation forms to gauge the performance, effectiveness, and reputation, of their Executive Director (ED) while undergoing a state-wide merger. Three evaluation forms are available below.

The first form is a one-page evaluation intended for judges, bar leaders, peers and other legal services program directors. This evaluation is a general form focusing on the ED's reputation and community relations.

The second form is three-page evaluation intended for CLS' Board of Directors and focuses on the ED's relationship with the legal community, his relationship with CLS' funding sources, his relationship with the Board of Directors, his work on financial and budget issues, and his work on the reorganization of legal services in Colorado.

The third form is a four-page staff evaluation. This form highlights the ED's leadership, his relations with staff, his handling of the program's reorganization, and his overall performance. To access the forms as PDF documents, click on the text below.
