The Boulder County office of Colorado Legal Services created survey forms in order to elicit feedback and attain outcome measurements on the services delivered to clients by CLS. Originally developed as a requirement by local funders, CLS created the four surveys available below with the intent of measuring how effective they were at providing access to the judicial system as well as determining if they were providing a service CLS' clients could not otherwise afford. Outside researchers provided them with assistance and conducted tests to maximize the effectiveness of the surveys. The researchers provide CLS with a Client Survey Handbook, which outlines the 8 steps involved in the collection of data through to the analysis of the result. An important feature is the final step "Linking Outcomes to Program Changes".

The "Client Survey", which is available in English or Spanish, is a survey given to the client while they are in the office requesting an evaluation of the services delivered to them by CLS. The "Client Follow-Up Survey" is a survey mailed to clients who received a referral to a CLS affiliated pro bono and staff attorneys in order to evaluate the service they were provided. The "Evaluation Survey for Attorneys" request CLS affiliated pro bono attorneys to provide feedback to CLS on the assistance they provided the client. The "Evaluation Survey for Referral Agencies" is a survey given to organizations that make referrals to CLS to determine the number of referrals provided by individuals in the agencies as well as to determine how they perceive the effectiveness of CLS.
